Why does a thick crease occur?
A line set too high, weak eye opening that leaves tissue beneath the line looking bunched, or a lid that is thick in itself can all be causes. In revision the cause is separated and the approach follows from it.
Does lowering the line leave more scarring?
The adhesion of the existing line must be released and the new line refined, and how it is handled varies with the earlier scar. Fine suturing aims to keep scarring down, though how well it heals varies from person to person.
In thick-crease revision, is ptosis correction also done?
Where weak eye opening leaves the area below the line looking bunched, the levator may be adjusted as well. It is not always combined; whether it is needed is decided after detailed assessment.
After thick-crease revision, how long does swelling last?
