The adhesion and fixation that formed the line are released and the lid tissue re-flattened so a crease does not set, aiming to return the eye area close to before. A line made by incision, however, carries a scar and tissue change, so returning it entirely without trace can be difficult.

The buried-suture method is approached by removing the thread and releasing adhesion, with less tissue change than the incisional method. Where the thread has been in place a long time, however, adhesion may have formed, and the degree of reversal varies with the state.

Because adhesion must be released and tissue refined, a fine mark can remain where the area was approached. Fine suturing aims to keep it down, but where a scar from the earlier line already exists, removing it entirely can be difficult.

The aim is to release the line and flatten the tissue, creating an impression close to before; but results vary from person to person with the lid and the earlier method. Confirming together at consultation what range is possible matters.

Once the tissue has settled, double-eyelid surgery can be considered again; but repeated operations can complicate the tissue, so careful assessment is needed. Timing and method are decided by the state of things.

Lowering the crease to soften the look and releasing the crease to return closer to your previous eyes are two different approaches. Which one suits you is decided in consultation, after reviewing your eyelid condition, the previous surgical method, and the look you want.

A crease made by the incisional method can also be approached by releasing the adhesion and fixation so the line no longer forms. Because scarring and tissue changes are already present, however, it may not be possible to return the eyelid completely without a trace, so we confirm the achievable range in consultation first.

Removing the sutures alone does not always release the crease. If the sutures were in place for a long time, adhesion will have formed at that spot and must be released as well, and how far that needs to go is judged from the condition of your eyelid.

Scarring and tissue changes from the previous crease can leave a faint mark. Once the tissue has settled, we look at the condition and decide whether further refinement is worthwhile; how much remains varies between individuals, so we go over the expected range in consultation.

As the crease is released and the eyelid tissue is flattened out, the area can look short of volume, so we add fat or tissue when needed. Whether reinforcement is required is decided after checking the thickness of the eyelid and the condition of the tissue.

Once the line is gone the overall impression changes, and your eyes may feel different from before. We first go over how the look will change after reversal during the consultation, and then set the scope to match the direction you want.

Right after surgery there is still swelling and tissue change, so it is too early to judge the final shape. It is usual to wait until the tissue has settled, and the timing is decided in consultation based on how your eyes are doing.
